Last night melded into this morning with more uncontrolled vomiting. We realized this afternoon that the THC we were putting into Tates G- tube likely wasn’t being absorbed well. We switched to a powder and as the day went on, it seemed to help significantly. Tate even ate a piece of toast tonight. Praying everything stays where it belongs! THC wasn’t initially something we wanted to use. However, we’ve exhausted all the anti emetics that Tate is able to use and things were just getting worse. We’re so grateful to have another option that is seemingly helping! There is a cannabis team here that serves as a guide, making sure there would be no interactions with her other meds. Grateful!!
Labs early this morning showed that Tate has an active CMV infection once again. She was positive with this prior to transplant, was treated and it cleared. They even chose a donor that had antibodies to cmv with hope that it wouldn’t reactivate. Despite antiviral meds prophylactically, it’s active again. This is a virus most adults carry. It’s not a problem in healthy immune systems and most never know they had it. It can wreak havoc in the immunocompromised. Currently Tate’s level is low. The hope is the current meds and her own body will keep it under wraps. They’ll continue to monitor it.
Labs also showed that Tates counts finally rose significantly overnight. They’ve been hovering above engraftment levels, not doing much. Seeing them increase was a reassuring positive this morning!
Pain team added a fentanyl patch today to work on decreasing the basal rate of the dilaudid. This is the easy part. The tough part comes when we wean the pca and bolus rate. Tate has to be able to handle oral meds before we can do that.
Really praying for some rest for Tate tonight with a calm tummy. Each day we hope and pray for improvement even in the smallest ways. It is a roller coaster, but I’m cautiously optimistic that we’re heading the right direction with the nausea.
